Prices for Medical Billing Services?

Can someone please help me with pricing for medical billing and coding services?

I would really appreciate a breakdown of reasonable charges.

Billing, Coding, Posting, Appeals.

Should they be:

Per claim? Per month? Percentage?

I would call a service and ask them like you are someone in business needing to use their service, it would be like getting an estimate for a job or something.....I am sure their are many different prices out there too........
